In addition, our Vision Service Plan offers a Laser VisionCare program designed to provide members with discounted laser surgery when obtained through the network.

Special Benefits for Women
In 1998, the federal government passed a Women's Health and Cancer Rights Act. This law requires that group health plans and health insurers providing benefits for a mastectomy must also provide the following: coverage for reconstruction of either or both breasts, and prostheses or treatment of physical complications in a manner determined by the doctor and patient.
OSI provides this health benefit.

Disability Coverage
Accidents will happen. And when they do, it's good to know OSI makes disability coverage available.
Our Short-Term Disability Coverage provides short-term income protection while you are disabled and unable to work due to sickness or non-work-related accidental injury. Our Long-Term Disability Coverage, which takes over where short-term coverage ends, provides a monthly benefit of up to 60% of your monthly earning.

Life Insurance
It's a very secure feeling to know that those who depend on you will have a measure of financial support should something happen to you. OSI is pleased to offer this reassurance of security at no cost to you.
Our Life and Accidental Death and Dismemberment (AD&D) coverage provides a lump-sum benefit upon the death of a covered individual. The benefit amount is doubled if death occurs as a result of an accident. In addition, to the basic benefit amount, you may (at your own cost) apply for additional insurance. This coverage is available for you, your spouse, and your children at group rates through payroll deduction.

Flexible Spending Accounts
How would you like to be able to use before-tax dollars to pay for dependent care and health-related services that you anticipate needing in the next year?
The Flexible Spending Account (also known as Dependent Care or Healthcare Reimbursement) allows you to do just that. By setting aside a portion of your salary through monthly payroll deductions you can pay for services not covered by your healthcare plan option and dependent care, including childcare and adult care expenses. Because you are paying with pre-tax money, you save tax dollars on those expenses.

Tuition Reimbursement and Scholarship Awards
OSI will pay 100% of tuition and books when an associate takes a course (through an accredited school or program) that is directily related to his/her position. And this reimbursement is paid up front, when you enroll, rather when you complete your course. All the company requires is that you attain a "C" grade or higher, and continue working at OSI for a certain length of time after you complete the course.
Because you're smart enough to work at OSI, your son/daugter could qualify for an OSI Scholarship award! The scholarship pays $1,000 per year for qualifying students to attend college. This award is paid prior to the start of the school year for four years, provided that the winner maintains a certain grade average, and his/her parent is an active, full-time OSI associate.

OSI University is about training you for your future. Any future. It's a wonderful way to develop skills - all kinds of skills - and advance your career through special OSI programs and courses, all across the country.
Locally, OSIU starts with extensive "new hire" training for account reps and supervisors. It leads to on-going skills and leadership development training for team leaders and managers.
Regionally, OSIU offers faculty in field offices to support that training with cohesive programs and course development.
Nationally, OSIU is a state-of-the-art training facility in St Louis. It provides high-quality training, education and other development resources to OSI associates everywhere.

401(k) Retirement Savings
If someone offered you a 50% match on your money, you'd either think it was too good to be true, or you'd say, "Lead me to it!" But that's exactly what you get with the OSI 401(k) plan, a very attractive way of saving for your retirement.
Simply designate a percentage of your salary to contribute to your 401(k) account through payroll deduction. The money you set aside does not get taxed before it goes into your account. As a result, your money can grow faster in the 401(k) than in many other types of savings or investment accounts. Furthermore, OSI matches contributions - up to a 50% match on your money up to a maximum 4% of your salary.

Leaves of Absence
OSI realizes that everyone at some time may have significant life circumstances that require our involvement, and we want to extend a helping hand when such events occur. This is why we offer associates the opportunity to take a leave of absence for certain compelling situations (medical leave, family leave, personal leave).
The amount of leave time you will be allowed depends on such things as how long you have worked for OSI and the type of leave you are requesting. In addition, OSI supports associates who request time off for funerals, jury duty, or military obligations.

Employee Assistance Program
Employee Assistance Programs ("EAPs") historically were dedicated exclusively to helping individuals address alcohol and drug abuse. And, while our EAP program continues to offer confidential assistance to associates or their family members who may need this help, our program has become a more extensive resource.
Our program is LifeBalance and it can provide you and your family access to confidential counseling that will help you deal with issues of daily living. What kinds of issues? Financial information, childcare, elder care resources, legal consultation, and other work or personal situations.
